Medical Office Building Tenancy:

1. Physician shall be an Approved Tenant of the Oasis Medical Office Building.
   
2. Physicians will be expected to meet minimal Credential Standards intended to enhance both quality of care and intraprofessional relationships.
3. Physician Participation will be by Invitation Only.

Physician Participates in Oasis Preferred Provider System:

1. Includes campus quality brochure featuring participating physicians, with Targeted Distribution.
2. Campus Website with doctor information and feature amenities.
3. Opportunity for enhanced participation in payer contracts.

Physician Places Deposit with Application:

• $10,000.00 Fee, for the cost of Basic Tenant Space Plan, Legal Fees and Processing staff time. Non-refundable, but Applicable to Lease cost.
• Payable to The Chaparral Group.
